Job Description The Case Manager assesses patients for the appropriate level of care required, both in the hospital and as part of the planning process for care after discharge. The Case Manager coordinates with patients, insurers and care Providers to confirm and/or assist in establishing the appropriate level of care for the individual patients. He/She informs patients of discharge planning options based on diagnosis, prognosis, resources and preferences related to home care, nursing home or community home services, and develops a safe and effective plan to meet the needs of each patient. Qualifications Graduate of an approved school of Nursing Current licensure as a Registered Nurse in the State of Vermont Mental and physical health sufficient to meet the demands of the position Advanced computer literacy; ability to learn and utilize electronic medical record
