SUMMARY: Provides management and leadership for Wellness team members and sets the standard for resident care at the community.


Essential Duties and Responsibilities

· Coordinates wellness program with resident, family, physicians, consultants and community team for total resident care.

· Assesses potential residents prior to move-in and residents returning from hospital or skilled nursing facility.

· Routinely visits residents to perform ongoing assessments and determine need for additional or modified services.

· Develops plans of action for nurses, medication technicians, and caregivers based upon resident need.

· Supervises recordkeeping of wellness and medical treatments and related services.

· Ensures availability of supplies and equipment related to wellness care.

· Monitors resident incident reports and medication error reports to assess causes and preventative measures.

· Trains all community team members in techniques developed to maintain resident independence and higher functioning. Documents training and conducts refresher training session to maintain appropriate skill sets. Evaluates team member skills by observation and implementing corrective action where necessary.

· Organizes and ensures completion of Community Quality Improvement Program, including but not limited to fall tracking, hospital readmissions, medication errors, etc.

· Acts as liaison between community Wellness team, other community teams, consultants, and physicians.

· Monitors Wellness budget, including labor.

· Ensures appropriate staffing for Wellness Department, schedules wellness staff on all shifts according to budget guidelines, and recommends changes to Executive Director.

· Recommends changes to Wellness policies and procedures to Executive Director and Vice President of Clinical Services.

· Participates in continuing education by attending in-services, seminars, and reading current literature.

· Studies and analyzes techniques in resident care, which will motivate and move residents towards greater independence, as well as higher levels of functioning.

· Acts as profession leader, role model and innovator in the community.

· Welcomes and interacts with newly arriving residents to achieve a successful transition.

· Keeps Executive Director informed of resident care activity, team member concerns, staffing patterns and other relevant topics.

· Assists the team with passing medications and provision of resident care as needed; May include picking up shifts due to limited availability of staff.

· Practice basic infection control with linens, hand washing, and in all other aspects of delivery of resident care.

· Attends mandatory meetings as scheduled by Executive Director.

· Maintains confidentiality of resident, community, employee, and company information.

· Maintains working knowledge of fire and disaster plans and responsibilities under emergency conditions.

· Performs other duties as assigned.


Supervisory Responsibilities

This position directly supervises the Wellness team at the assigned community and may supervise the Memory Care team. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. The Executive Director must be consulted before disciplinary action is taken.


Qualifications

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.


Must satisfy all state health, licensure/certification, and background requirements.

Education/Experience: Must have a high school diploma or equivalent plus 5 years of experience in nursing techniques, including 2 years in long term care. Prefer assisted living experience. Demonstrates leadership skills in applying nursing and rehabilitative care techniques and has in-depth knowledge of applicable state and Federal rules, regulations, and standards.

Certificates and Licenses: Currently licensed or eligible for license as LPN or RN, plus any additional licensing requirements in state(s) of practice.

Communication Skills: Receive and carry out verbal or written instruction. Communicate effectively in person, over the phone, or via email with customers and co-workers. Write and proofread routine correspondence and reports with appropriate grammar and spelling. Make group presentations to share information or explain policies and procedures.

Organization/Self-Management Skills: Perform a variety of duties, moving between tasks of differing nature, with occasional distractions and interruptions. Prioritize competing requirements to complete work in a timely manner when there are changes to workload and assignments. Work independently and follow through on assignments with minimal direction.

Computer Skills: Proficiency with computers, Windows operating system, and Microsoft Office Suite.

Physical Demands

Constantly stands and walks around the community, interacting with residents, coworkers, and others. Uses hands and fingers to feel, handle, and operate supplies, controls, tools, and objects, and hands and arms to reach. Frequently communicates with others to exchange information in person, and over the phone, and must be capable of effective communication in these situations. Occasionally moves items weighing up to 25 pounds and may assist in transferring residents weighing more than 100 pounds. Occasionally required to bend, twist, stoop, kneel, crouch, or crawl.

Work Environment

This position is most frequently situated inside of a climate-controlled assisted living community, with a low to moderate noise level. Staff working directly with residents can be exposed to communicable diseases and infections, other biological hazards, and various drugs and chemicals.


Travel

Up to 10% local travel required.
