SUMMARY OF QUALIFICATIONS

1.                  Current license as a Licensed Practical Nurse in the State of Ohio required.

2.         Is a graduate of an accredited school of Licensed Practical Nursing.  

3.                  Successfully completes facility conducted orientation, mandatory training and in-service programs.

4.         Previous long term care experience is required.  Supervisory experience and organizational skills in a long term care setting preferred.

5.                  Must meet health assessment requirements, including two stage Mantoux skin test.

6.         Is listed in good standing as confirmed on the Ohio State Department of Professional Credentialing.

7.                  Possesses excellent technical assessment and documentation skills, and leadership quantities.

8.         Must be capable of performing the essential functions of the job, with or without reasonable accommodations.

9.                  Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required
