Lakeland Health Care Center located in Haskell, New Jersey, a quality skilled nursing facility, offers short and long-term nursing and rehabilitation care.

SIGN ON BONUS $5,000 FOR FULL TIME

Full time/Part time/Per diem

7-3/3-11/11-7 All shifts available 

Duties/Responsibilities

• The primary job duty is to provide basic medical attention to patients and assessing their needs.

• Day-to-day coordination and oversight of the nursing process for residents assigned to care.

• Direct and indirect provision of care according to each resident's nursing and medical plan of care and in accordance with state and federal regulations and accepted nursing standards of practice.

• Assessing, observing, and evaluating resident’s clinical condition, document findings in resident electronic medical record and notify the provider and/or RN when necessary.

• Updating or modifying care plans as needed to reflect any changes to a resident’s care and service needs.

• administering medications timely, accurately and according to orders.

• Participating in meetings related to resident outcomes and care.

• Responding to residents, family, or responsible party in a timely and professional manner.
