Job Description

The LPN On Call would be on call from Friday at 4:30 pm until Monday at 8:00 am (eastern). Under the supervision of the Clinical Director or designee, the LPN would be responsible to cover as-needed visits that arise, as well as scheduled weekend visits. Compensation will include an on-call stipend PLUS pay per visit for each visit completed. The LPN will be responsible for evaluating patient health problems and needs, following nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention. Licensing or registration required. 

ESSENTIAL DUTIES AND RESPONSIBILITIES 

The essential functions include, but are not limited to the following: 

  • On call from Friday at 4:30 pm until Monday at 8:00 am (eastern)
  • Must cover any as-needed visits that arise, as well as schedule weekend visits
  • Provide skilled nursing care under the direct supervision of the Clinical Supervisor
  • Complete evaluation tasks, including vital signs and medication reviews
  • Coordinate with physicians, therapists, and other individuals in the client’s care plan
  • Evaluate and chart observations of the client’s condition at each visit
  • Maintain complete and accurate documentation of nursing care
  • Work collaboratively with all members of the health care team
  • Provide education to clients and families on proper home health care procedures and strategies
  • Record patients' medical information and vital signs.
  • Monitor, record, and report symptoms or changes in patients' conditions.
  • Consult and coordinate with healthcare team members to plan or evaluate patient care plans
  • Monitor all aspects of patient care, including diet and physical activity
  • Evaluate the needs of individuals, families, or communities, including evaluation of individuals' home or work environments, to identify potential health or safety problems
  • Perform other duties as assigned

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES) 

  • Must be available on weekends from Friday 4:30 pm until Monday 8:00 am (eastern)
  • High School diploma is required. Graduate of an accredited state approved LPN program required.
  • Current and valid Connecticut Licensed Practical Nurse license
  • One year of LPN experience
  • Current CPR certification (required)
  • Driver’s License, Car Insurance & Registration required
  • Strong interpersonal skills that build personal rapport and establish trust with the client
  • Computer literate & Strong organizational skills
  • Excellent written and communication skills
  • Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)
  • Home health care experience
  • Bilingual (Spanish) preferred
