The RN Coordinator is responsible for planning and implementing the Behavioral Health Program on the unit. He/she supervises the Nursing staff and the Social Services on the unit and assists in developing Facility Policies regarding participating in therapeutic group programs, individual counseling and community planning for health and welfare services. The Coordinator  plans and assists in research projects and is responsible for assisting with discharging planning rom the Behavioral Health Program.

Job Responsibilities:

1. Plans and Directs comprehensive therapeutic program which provides counseling and other supportive services for residents and families;

2. Consults with the Administrator and other department heads regarding interdisciplinary issues;

3. Works directly with the residents and families experiencing personal and environmental difficulties or concerns related to the resident’s physical, emotional and psycho-social needs;

4. Assures that medically related therapeutic services are provided to maintain and improve each resident’s ability to control everyday needs(e.g., appropriate adaptive equipment for eating, ambulating, etc.) and mental and psycho social needs(e.g., sense of identity, coping abilities, and sense of meaning or purpose);

5. Maintains a written record of the frequency and nature of consultation and  therapeutic program interaction and response;

6. Performs an evaluation of each resident’s psycho social needs. The plan for providing care shall be formulated and recorded in the residents; medical record and periodically re-evaluated in conjunction with the resident’s total plan of care;

7. Collects pertinent data upon admission of each resident in the Behavioral Health Program to be placed in the medical record, including information about the personal and family problems related to the resident’s illness and care, and actions taken to meet the resident’s needs. Pertinent information shall be made available to the attending physicians and other appropriate staff members;

8. Assist each resident with adjustment to the social and emotional aspects of placement, by including clearly defined goals and interventions in the plan of care;

9. Coordinates the Behavioral Health Program interdisciplinary care plan team and schedules conferences in a timely manner and invites residents/families members as appropriate;

10. Acts a s a liaison between residents, families and outside agencies, and the facility administrator to ensure the residents rights are maintained; 

11. Assumes the role and responsibility of the Admission Coordinator in determining

       resident’s criteria into the Behavioral Health Program along with the Director of Nursing and the

       Administrator;

12. Functions in a management capacity and adheres to all policies and procedures of the facility as a representative of the Facility’s administration;

13. Appropriately manage resident’s behavior and guide resident towards more acceptable social behaviors

14. Assists each resident to achieve and maintain an optimum level of function and self-care through education and retraining in the activities of daily living.

15. Implement positive and appropriate behavioral interventions.

16. Participate and provide input into resident’s plan of care and ensure interventions are followed. 

17. Outdoor supervision of recreational activities and smoking safety.

18. Complete daily behavioral documentation for each resident assigned.

19. All other duties as assigned

Job Requirements:

1 Training and certification in Crisis Prevention Intervention and the ability to demonstrate advanced behavioral techniques.

2 Experience in working with complex and/or severe behavioral residents and applying therapeutic techniques to deescalate the resident.

3 Experience in leading various educational and therapeutic program topics related to, but not limited to ADL care, hygiene, social skills, coping skills, anger management, 

4 Ability to recognize escalating aggressive behaviors and providing calm redirection to a controlled, safe environment.

5 Ability to work with mentally ill residents having special needs and/or aggressive behaviors

6 Knowledge of behavioral interventions and applied behavioral analysis.

7 Knowledge of behavior management strategies and techniques related to residents experiencing behavioral difficulties.

8 Knowledge of aggressive behavior characteristics pertaining to mentally ill residents.

9 Minimum experience working with behavioral residents preferred: 3 years.

10 The RN Nursing Supervisor must be a graduate of an accredited school of nursing and there must be current registration with the professional board of nurse examiners of NJ.

11 Experience in the nursing field.

12 Supervisory experience in long term care is required.
