Job Description

Rehabilitation Supervisor overall manages the activities of Rehabilitation services ,monitors the performance of the therapy staff and supervises the delivery of therapy programs to the patients. 

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

  • Provides direct therapy/rehabilitation patient care services as needed.
  • Supervision of therapy team members and the oversight of quality of patient care to meet federal, state and local regulations and the Company’s policies 
  • Assists in the interviewing, hiring and separation of direct reports 
  • Evaluates clinical performance of direct reports and recommendations about patient care
  • Participates in the Agency Quality Committee and review of monthly, quarterly, and annual reports related to QAPI Program efforts
  • Monitors clinical documentation for completeness and accuracy and assures that it meets quality standards for good clinical documentation and successful billing
  • Coordinates and participates in educational staff performance improvement in-services. Assists with orientation and in-service training for department staff members. Participates in guidance and educational programs
  • Participates in required clinical team conferences with other home health staff
  • Demonstrates knowledge and proficiency of Physical Therapy modalities
  • Interacts effectively with corporate staff, managers, employees, clients, professional colleagues and vendors
  • Consults other departments, as appropriate, to collaborate in patient care and performance improvement activities
  • Coordinates and conducts regular staff meetings to disseminate new department information, educate therapy staff on new trends in therapy and to hear issues in department and plan solutions 
  • Performs other duties as assigned

Minimum Qualifications (Knowledge, Skills, and Abilities)

  • Graduation from an approved Physical Therapy curriculum
  • Current and valid Connecticut Physical Therapist license (required)
  • Experience in Home health is preferred
  • Driver’s License, Car Insurance & Car Registration
  • Strong clinical assessment, organizational and communication skills
  • Ability to handle multiple assignments simultaneously 
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation
  • Bilingual (Spanish) preferred
  • Strong interpersonal skills and good judgement
  • Must have the ability to work well under pressure and stay calm and focused in emergent situations
  • Be highly self-motivated and results-driven
