Job Details

Wellness Manager is responsible for coordinating, training, and supervising Wellness Center Care Staff in accordance with state regulations and The Forum policies and procedures. Responsibility also includes management of all the Wellness Center functions and working collaboratively with all departments to ensure the safety and wellbeing of all residents and team members.

The Wellness Manager is responsible for evaluating the independent living needs of residents, assisting with the development of programs to meet those needs, teaching self help techniques to ensure that the medical, social, and emotional needs and the general welfare of residents is met. Participates in the decision making process regarding resident admissions and transitions to the Health Center.

QUALIFICATIONS:

An RN licensed by the State of California. Two years working in geriatrics in long term care, home health or other health care setting. Strong administrative and leadership skills; the ability to participate in direct clinical care and decision making, knowledge of geriatric, clinical and psycho-social best practices; advocacy and care coordination skills and the ability to re-prioritize daily tasks in order to accommodate fluctuating needs of the residents and the Community. Must be able to communicate effectively with residents, families, staff and community State officials and the general public. Must understand RCFE regulations and hold a valid RCFE certificate in the State of California. Must pass criminal background check.
