Currently seeking a skilled and mission driven Admissions Liaison to join our ever-growing team!

Day in the Life of a Admissions Liaison

• Visiting Patients, Discharge Planners and clinicians at multiple sites across the city
• Providing onsite patient assessments
• Reviewing Clinical Patient reports
• Complies with state and Federal Regulations.
• Works Collaboratively with members of the healthcare team.
• Serves as a primary coordinator between initial patient contact and facility
• Assesses and coordinates patient's transfer and planning needs with members of the healthcare teams.

Requirements:

• Consistent and Reliable Transportation
• Must be a self-starter able to work independently and efficiently
